Military Legal Assistance Attorney Downloadable Application Forms

Before you begin, read all instructions below. Print out the page and keep for future reference.

To download the forms, look for the load VBBE Forms for Macintosh (OSX Only) icon below and download for Windows or Mac. No special software is required.

After you download, you will be asked a series of questions. Your answers will determine the forms you are required to provide.

Contained in the Download are forms you will complete using the VBBE Forms Wizard and links to PDFs that may be required. The downloadable forms include all Applications, Character & Fitness Questionnaire, Character & Fitness Update, and other miscellaneous required forms for the following types of admissions:

Helpful Hints for the VBBE Forms Wizard

  • The VBBE Forms Wizard will step you through the process in a very efficient manner and will assist you by helping you complete the questions easily.
  • Once you have downloaded the forms, the easiest way to navigate the forms is by using the VBBE Forms Wizard. Simply click the Next Question button in the upper right corner of the screen and the VBBE Forms Wizard will do its magic.
  • If you prefer, you can trigger the Next Question by using the ENTER key on your numeric keypad.
  • If you are using a laptop that does not have a numeric keypad, determine what your laptop keyboard uses to emulate the Enter key. (Some Laptops use the FN and the RETURN keys together to emulate the ENTER key.)
  • Please note that this is NOT the RETURN key. The RETURN key is used to create a carriage return in text fields.
  • If you are required to complete a separate PDF form, please use ADOBE ACROBAT READER (or suitable PDF reader) to complete them.

VBBE Forms Wizard for Windows®

  SPECIAL NOTE FOR VISTA & WINDOWS 7 USERS: Once the installation is complete, you MUST right click the VBBE Forms Wizard v3.exe icon and choose "Run as administrator" each time you access the VBBE Forms Wizard. Download Instructions (Windows): Depending on the internet browser you use, the instructions for properly installing the VBBE Forms Wizard may differ. Here are the Internet Explorer instructions:
  • Please download by right clicking on the Download link and choosing Save (Do Not choose RUN as this will NOT install the Wizard properly).
  • Save this to a location on your computer where you can easily locate the file, your desktop, download folder, or my documents folder.
  • When the download is complete, locate the file"VBBEWizardv3PC.exe" and double click. This will install the VBBE Forms Wizard on your computer and place an icon on your desktop.
  • Double click the icon, VBBE Forms Wizard v3.exe, located on your desktop. Please note: This may take several minutes.

VBBE Forms Wizard for Macintosh

SPECIAL NOTE Macintosh has 2 versions: To determine which version of the Macintosh OS you are using, click the "Apple" icon in the upper left hand corner of your menu bar and choose "About this Mac". The "Version" is located under "Mac OS X". Choose correct MAC download for either Version 10.5.x or greater or Version 10.4.x.
  Download Instructions Macintosh OSX 10.5.x and greater: Depending on the internet browser you use, the instructions for properly installing the VBBE Forms Wizard may differ. Here are the Safari instructions:
  • Please download by clicking the Download link above.
  • When the download is complete, double click VBBEWizardv3Mac.dmg to mount the installer disk.
  • When the installer disk mounts, double click VBBEWizardMac to begin the installation process.
  • It will install the VBBE Forms Wizard v3 folder to the Desktop.
  • Once the installation is complete, open the VBBE Forms Wizard v3 folder located on your desktop and double click the icon VBBE Forms Wizard v3 to open the VBBE Forms Wizard v3. Please note: This may take several minutes.

  Download Instructions Macintosh Version OSX 10.4.x: Depending on the internet browser you use, the instructions for properly installing the VBBE Forms Wizard may differ. Here are the Safari instructions:
  • Please download by clicking the Download link above.
  • When the download is complete, the installer will automatically open in a window with a folder, VBBE Forms Wizard v3.
  • Simply drag the VBBE Forms Wizard v3 folder to the Desktop to install it on your desktop.
  • Once the install is complete, open the VBBE Forms Wizard v3 folder located on your desktop and double click the icon VBBE Forms Wizard v3 to open the VBBE Forms Wizard v3. Please note: This may take several minutes.

SPECIAL NOTE Macintosh Version OSX 10.4.x: If the installer does not automatically open, locate the file VBBEWizardv3.dmg, depending on your browser preferences, this may be on your desktop or downloads folder. When you locate the file, double click. This will open the installer file. Then follow the instructions above.
